This Sliding Fee Scale only applies to Bodywork with Bevin Keely, and takes effect beginning in September 2021.
The rate you pay is an agreement we make as two humans living in an expensive city with bodies to take care of and other financial obligations to uphold. It is my desire to be of service to folks who need bodywork, and it is my reality that I have to charge for it in a way that sustains me. I have set my rates at a place that will do so, but in an effort to be of service to a broader spectrum of people in my community I will work on a sliding scale when necessary. First:
- Do you have health insurance that will pay for out of network *massage benefits with the right paperwork? (*All of my work is “massage” to the insurance companies.)
- Do you have a Health Savings Account (HSA) or Flexible Spending Account (FSA) you can use?
- Can you see me at longer intervals or do a shorter session? If you are used to a monthly session at $95, could you do every six weeks at $120? If you usually do 90 minutes, would 75 work?
- We can set up a payment plan to spread out the cost of the work you receive. This makes the most sense for work done in a series, but could also work for regular sessions.
- If you have something to trade or barter with, let’s talk.

If the lowest number above is still not accessible to you, please say so, there may be some money available from my tip-it-forward fund to sponsor your bodywork.

At this time Bevin is taking new clients within the First Choice Health Network (Kaiser’s extended network). All insurance sessions can be booked from the regular service menu, including L&I and PIP claims.
For all clients billing insurance of any kind, we ask that you have a prescription for massage from your doctor, regardless of whether your plan says you need one. Your prescription must include the proper ICD-10 diagnosis code/s and the number of sessions prescribed. Please note: our correspondence address is not our physical location address, and we do not have a fax machine. For correspondence address please ask your bodywork practitioner.
If your health insurance offers out-of-network massage benefits we can furnish you with the proper paperwork to get our work reimbursed at the out-of-network rate if you wish.
PIP and L & I claims will require that we send along our treatment notes with billing. We will ask you to sign this form:

Schedule online with the BOOK APPOINTMENT button above. If you don’t see anything available we are fully booked for the next 90 days. If you want to do a series of SE or SI work with Bevin and nothing is available, please get in touch. There is also a cancellation list you can join at the bottom of the page.
New clients please fill out the intake form that the booking system sends you. It will reference this HIPAA form and this Notice of Privacy Practices referenced within the HIPAA form.
Gift Certificates are available, priced the same as regular treatments HERE.
We accept payment through a variety of apps, cash, checks, most major credit cards, and do work with some health insurance companies, as well as L & I and automobile insurance claims. If you have a Health Savings Account (HSA) or a Flexible Spending Account (FSA) our services may qualify as an eligible expense.
Insurance Reimbursement: Massage therapy can be billed through your insurance if it is considered to be medically necessary For more details on using your insurance to pay for your time with us, please see FAQ.
